NURSE JOB DESCRIPTION

POSITION DESCRIPTION:

The Nurse will provide complete individual and consultant satisfaction, while maintaining clinical compliance.

REPORTABILITY: Chief Executive Officer

QUALIFICATIONS:

  • Must be a registered or licensed nurse in the state of Indiana
  • Must have at least two (2) years’ nursing experience
  • One (1) year developmental disabilities experience desirable
  • Must be at least 21 years old
  • Must have a current nursing license

RESPONSIBILITIES:

  • Implement, supervise and monitor clinical compliance
  • Completion of the State’s Risk Assessment Tool
  • Development, oversight, and maintenance of Wellness Coordination PlanXXX
  • Development, oversight, and maintenance of the risk Plan which includes:
    • Training of Direct Support Professionals to ensure implementation if Risk Plans
    • Consultation with the individual’s health care providers
    • Face to face consultations with the individual as described in the support plan
    • Consultation with the individual’s support team
    • Active involvement at all team meetings, reporting on the Wellness Coordination Plan as it relates to the individual’s full array of services as listed in the ISP
  • Monitor the health and safety of the individuals receiving services
  • Complete record reviews to include, but not limited to, review of physician’s orders, review of labs, review of medication, review of MAR, review of records, and individual specific monitoring
  • Collaborate with the Manager and Administrator regarding findings and recommendations pertaining to the health and safety of individuals receiving residential services
  • Implement, monitor and enforce clinical quality assurance policies and procedures
  • Provide in-house clinical consulting
  • Ensure appropriate delegation, clinical training, and supervision of DSP’s who perform duties such as medication administration and other nursing tasks
  • Other duties as assigned by the Chief Executive Officer.

DIRECT SUPPORT PROFESSIONAL (DSP) JOB DESCRIPTION

POSITION DESCRIPTION: The Direct Support Professional or DSP will assist the individuals we serve in achieving the quality of life each person desires and encourage mutual respect and dignity through teaching, guidance and counseling in accordance with Executive Home Care philosophies, local, state and federal regulations and management policies.

REPORTABILITY: Program Director

QUALIFICATIONS, SKILLS, KNOWLEDGE AND ABILITIES:

  • The ability to provide individualized support to the people we serve.
  • The ability to effectively communicate verbally and in writing.
  • The ability to work effectively with minimal supervision.
  • The ability to solve problems according to policies and procedures.
  • The ability to support the individuals in meeting their financial needs.
  • The basic knowledge of cooking and cleaning.
  • The ability to safely lift up to fifty pounds and transfer people with mobility limitations. High school diploma or equivalent required.

REQUIREMENTS:

  • Must have valid Driver’s License and be at least 18 years old
  • Must maintain First Aid and CPR certification
  • Must have a negative TB test prior to being offered employment
  • Criminal history must be free from ALL felonies
  • Must have verified US residency
  • Must have a highschool diploma or GED
  • Must complete DDRS requires areas of training on a regular basis, at minimum annually (including abuse, neglect, exploitation, incident reporting, CORE A and B (medication administration and side effects of), and physical intervention techniques needed for emergency behavioral supports)
  • Must be proficient in Person Centered Planning, Individual rights, respecting dignity of an individual, choice, competence, community presence and participation, health and wellness,communication, and additionally required individual specific interventions as warranted

    • To follow policies and procedures outlined by Executive Home Care manuals and in-service training about the individual’s rights.
    • To uphold Executive Home Care philosophies, including individual-centered services, social role valorization, normalization, and independence.
    • To comply with state and federal regulations concerning individual rights and endangered adult issues.
    • Ensure that the individual’s home is personalized according to his taste and means as much as possible.
    • To ensure each individual receives support in maintaining an appearance that enhances his social acceptance by the individual’s tastes and means.
    • To ensure that community-based, age-appropriate, meaningful activities occur regularly.
    • To encourage correspondence and contact with family and friends.
    • To ensure transportation is provided.
    • To ensure self-governing meetings are held regularly.
    • Ensure that all on-call staff are aware of significant changes in individuals’ behavior and health status.
    • To communicate pertinent information to appropriate team members.
    • To involve individuals in programming decisions, facilitating individuals’ control and choice.
    • Complete assessments by incorporating the information and ensuring that this information is provided to all team members.
    • To implement programs following Team decisions.
